By submission of this signed contract the artist waives all claims against the City of North Olmsted, Olmsted Historical Society Rocky River Reservation, The Cleveland Metroparks, the Kiwanis Club and Northcoast Promotions, Inc. for loss or damage. The artists agrees to abide by the requirements and rules stated in the 2009 entry form. I fully understand all aspects of the show and have enclosed my entry fee.

The Arts & Crafts Show is sponsored by the Kiwanis Club of North Olmsted and the City of North Olmsted
Cultural Activities Commission.
Northcoast Promotions, Inc. has been commissioned to help find artists for this event.
Participation
All arts & crafts will be considered. All work must be original by the artist. Commercial items or kits will not be
accepted. Artists are to sell only items that they have created.
Submission
The Kiwanis Club of North Olmsted and the City of North Olmsted Cultural Activates Commission will jury all
applications.
Submit at least 3 images on CD ROM, slides or photos of work & I image of booth set up.
Label each photo with your name & telephone number.
Jury
On the first Wednesday of each month beginning in April, the committee will meet to jury applications submitted. The jury process will continue until all categories are filled. The number of artists will be limited in each category. Entries will be accepted on the basis of presentation and quality.
Notification:
Artists who enclose a self addressed, stamped envelope with enough postage to return materials submitted will receive
notification of their acceptance or denial with the show.
Applicants will be notified by the 20th of each month of submissions after jury.
Payments
Application & booth fees are due between March 2 and July 6, 2010.
Booth fees will not be deposited until exhibitor is notified & accepted into the event. Booth fees will be returned to
exhibitors not accepted into this show.
Withdrawals
Refunds or show credits will not be granted once accepted into this show.
Taxes & Insurance
Sales tax is the responsibility of the artist.
Artists are responsible for acquiring the necessary insurance for damage and theft.
Displays & Space
Artists will receive a 15’ x 15’ tent space.
Artists are to display in a professional manner.
Artists should provide appropriate boxes/bags for customers to carry purchased artwork.
Exhibitors are responsible for providing their own booths, tables, chairs and other props unless indicated otherwise on
this form.
Exhibitors are asked to keep their area clean of debris.
Space Assignment
Directions and other information will be enclosed with your acceptance letter.
Security
Frostville, The Kiwanis Club of North Olmsted & Northcoast Promotions, Inc. assume no responsibility for loss or damage. We suggest that you carry your own insurance. Metroparks Police will be present periodically throughout the show.
